Thank you for taking the time to sign up for piano or guitar lessons at KairosPDX Learning Academy. Classes will begin with the new school year: September 2025, exact date to be determined.

If your student was enrolled with Play It Forward last year, please fill out this form instead.

Family Commitment

We look forward to partnering with you to support your student's musical journey. We ask that families join us in supporting their Play It Forward student by agreeing to:

1. Receive and maintain an instrument which will be delivered to your home for free
2. Ensure that your student attends weekly lessons in school
3. Ensure that your student plays their instrument at home at least 3 times per week
